B12 Deficiency Symptom Library

Comprehensive, evidence-based guide to 122 direct symptoms across 6 body systems, with separate sections for associated conditions and treatment-response items.

Direct Symptoms vs Related Clinical Items

This library separates direct symptoms of B12 deficiency from other clinically relevant items that should not be presented in the same way.

Direct Symptoms of B12 Deficiency

These are symptom patterns and clinical manifestations that can occur as part of B12 deficiency itself.

Associated Conditions

These are not presented as direct symptoms caused by B12 deficiency. They are listed separately because they may co-occur, overlap clinically, or be relevant when discussing B12 status.

Treatment-Response Items

These describe symptoms or effects that can appear during treatment or early recovery, not untreated deficiency itself.

Associated Conditions

13 listed

Use these as prompts for a more careful clinical conversation, not as proof of B12 deficiency.

Recurrent Urinary Tract Infections

Recurrent Urinary Tract Infections is a reported feature in vitamin B12 deficiency, often affecting reproductive & urinary. Persistent, progressive, or function-limiting symptoms should be reviewed clinically.

Restless Legs Syndrome

Restless Legs Syndrome is a reported feature in vitamin B12 deficiency, often affecting lower limbs. Persistent, progressive, or function-limiting symptoms should be reviewed clinically.

Splenomegaly (Enlarged Spleen)

Splenomegaly (Enlarged Spleen) is a reported feature in vitamin B12 deficiency, often affecting abdomen. Persistent, progressive, or function-limiting symptoms should be reviewed clinically.

Vitiligo

Vitiligo is a reported feature in vitamin B12 deficiency, often affecting general system. Persistent, progressive, or function-limiting symptoms should be reviewed clinically.

Treatment-Response Items

11 listed

These can appear after treatment starts and should not be confused with the core untreated deficiency symptom list.

Wake-up Chills/Fever/Malaise

General disorders listed in SmPC include fever, chills, and malaise.

Wake-up Dizziness/Light-headedness

Reported by both SmPC and NHS side-effect pages.

Wake-up Gastrointestinal Upset (Nausea/Vomiting/Diarrhoea)

Nausea, vomiting, and diarrhoea are listed in SmPC and NHS guidance.

Wake-up Headache

Listed in SmPC and NHS side-effect guidance for hydroxocobalamin.

Wake-up Hot Flushes

Hot flushes are listed in SmPC and NHS side effects.

Wake-up Injection-site Reaction

Injection-site reactions are common in both SmPC and NHS patient guidance.

Wake-up Palpitations/Irregular Heartbeat

NHS advises urgent assessment for irregular or newly noticeable palpitations after treatment.

Wake-up Paraesthesia (Pins and Needles)

Neurological adverse effects in SmPC include paraesthesia.

Wake-up Possible Hypokalaemia Symptoms

Product labels advise potassium monitoring early in treatment; review literature notes historical association and cautions on causality.

Wake-up Skin Eruption (Acneiform/Bullous/Rash)

SmPC lists acneiform and bullous eruptions; NHS lists allergic skin reactions such as rash or hives.

Wake-up Tremor/Shakiness

Tremor is listed as a neurological undesirable effect in SmPC.

General System

Whole-body signs that may reflect systemic effects of vitamin B12 deficiency.

Moderate Priority
35 symptoms
General symptoms are common but non-specific, so pattern recognition matters.

Head, Neck, Throat & Spine

Neurological and neuropsychiatric features affecting cognition, sensation, and central pathways.

High Priority
64 symptoms
Neurological signs require prompt assessment due to risk of persistent deficits.

Chest & Upper Limbs

Cardiorespiratory, sensory, pain, and movement features affecting the chest and upper limbs.

Moderate Priority
20 symptoms
Mixed vascular and neurological features can overlap in this region.

Abdomen

Gastrointestinal and abdominal manifestations, including sensory and organic findings.

Moderate Priority
10 symptoms
Digestive and abdominal findings may indicate both symptom burden and underlying cause.

Reproductive & Urinary

Reproductive and urinary manifestations, including autonomic and pain-related symptoms.

Moderate Priority
9 symptoms
Evidence strength varies; clinical context and differential diagnosis are essential.

Lower Limbs

Sensory, gait, balance, and movement symptoms affecting mobility and function.

High Priority
18 symptoms
Progressive gait or balance problems should be escalated early.

Important Clinical Notes

Symptoms Without Anaemia

Neurological and psychiatric symptoms can occur BEFORE blood abnormalities develop. A normal blood count does not exclude B12 deficiency.

Symptom Overlap

Many B12 deficiency symptoms overlap with other conditions. Getting a proper diagnosis requires blood tests, not just symptom assessment.

Treatment Response

Some symptoms (particularly neurological) may take months to improve with treatment, and some damage may be permanent if treatment is delayed.
